The Biggest Mistakes People Make After Ozempic®, Wegovy® and Mounjaro®

A Plastic Surgeon’s Perspective on Protecting Your Results After Major Weight Loss

Over the past few years, medications such as Ozempic® (semaglutide), Wegovy® (semaglutide) and Mounjaro® (tirzepatide) have transformed the treatment of obesity. For many patients, they have been life-changing, allowing significant weight loss after years of frustration.

As a plastic surgeon, I have also witnessed another side of this remarkable success.

While patients celebrate their healthier weight, many are surprised by the changes that occur in their face, breasts, abdomen, buttocks, arms and thighs. Some of these changes are unavoidable. Others, however, can be minimized—or even prevented—with proper planning.

Here are the biggest mistakes I see patients make after losing weight with GLP-1 medications.

Mistake #1: Losing Weight Too Quickly

One of the greatest misconceptions is that faster is always better.

Rapid weight loss often means the body doesn’t have enough time to adapt. 

The result may include:

  • increased skin laxity 
  • greater “skin envelop” (eg. face, breast, buttocks, thighs, arms, etc.) deflation 
  • muscle loss 
  • nutritional deficiencies 
  • fatigue 
  • poorer surgical healing later on 

The goal should not simply be reaching your ideal weight.

It should be reaching it while preserving muscle, skin quality and overall health.

Mistake #2: Ignoring Protein Intake

This is probably the most common mistake.

GLP-1 medications dramatically reduce appetite.

Unfortunately, many patients eat too little protein, causing the body to lose valuable muscle along with fat.

Muscle loss contributes to:

  • slower metabolism 
  • weakness 
  • poorer posture 
  • worse body contour 
  • increased skin sagging 

Aim for adequate daily protein intake (often 1.2–1.6 g/kg/day, depending on your medical situation and activity level), ideally under the guidance of your physician or dietitian.  Aim for eggs, nuts, seeds, legumes, cottage cheese and leaner types of proteins such as fish, chicken and lean beef cuts.

Mistake #3: Never Doing Resistance Training

Walking is wonderful.

It is simply not enough.

Weight loss without resistance exercise is frequently accompanied by loss of lean body mass.

Strength training helps preserve:

  • muscle 
  • bone density 
  • metabolism 
  • body shape 
  • surgical outcomes 

Even two or three sessions per week can make an enormous difference.

Mistake #4: Waiting Too Long Before Seeing a Plastic Surgeon

Many patients believe they should wait until years after weight loss.

In reality, consultation can be helpful before reaching goal weight.

A plastic surgeon can discuss:

  • expected skin changes 
  • likely facial volume loss 
  • breast changes 
  • body contouring options 
  • timing of procedures 
  • strategies to optimize skin quality 

Planning early often produces better long-term outcomes.

Mistake #5: Expecting the Skin to “Bounce Back”

Unfortunately, skin has biological limits.

Its ability to contract depends on:

  • age 
  • genetics 
  • amount of weight lost 
  • smoking 
  • sun damage 
  • previous pregnancies 
  • duration of obesity 

Creams, supplements and collagen powders may improve skin hydration or texture to a very modest degree, but none can remove significant excess skin.

Patients deserve realistic expectations.

Mistake #6: Stopping the Medication Too Soon

Some patients discontinue therapy immediately after reaching their goal weight.

Unfortunately, obesity is a chronic disease.

Research consistently shows that many patients regain substantial weight after stopping GLP-1 medications, especially without a long-term maintenance strategy involving nutrition, exercise, and medical follow-up.

The decision to continue or stop medication should always be individualized with your prescribing physician.

Mistake #7: Underestimating “Ozempic Face” and “Ozempic buttocks”

The media popularized the expression “Ozempic Face” and “Ozempic buttocks”, but the phenomenon has little to do with the medication itself.

It is primarily the result of rapid fat loss.

Patients may notice:

  • hollow cheeks 
  • temples becoming sunken 
  • more visible jowls 
  • deeper folds around the mouth 
  • neck laxity 
  • an older appearance 
  • deflation of buttocks
  • sagginess of buttocks

Ironically, many patients become healthier while feeling they suddenly look older.

And it affects men too; their gynecomastia (so called “men’s boobs”) gets saggier, buttocks deflate and sag, neck skin and jowls also descend and they will look older).

Fortunately, several options can restore youthful facial contours.

Mistake #8: Believing Fillers Are Always the Answer

Facial fillers certainly have a role.

However, after major weight loss they are often not the complete solution.

Some patients actually require:

  • fat grafting 
  • eyelid surgery 
  • neck lift 
  • facelift 
  • skin resurfacing 
  • skin excision (arms, thighs, buttocks, etc.)
  • sometimes implants (chin, breasts, buttocks, pectoral)
  • combination treatments 

The best treatment depends on whether the problem is volume loss, skin laxity, or both.

Mistake #9: Having Body Contouring Surgery Too Early

Timing matters.

Ideally, weight should remain relatively stable for approximately 6 months before major body contouring surgery especially when skin excision is needed (facelift, arm lift, thigh lift, traditional buttock lift, gynecomastia surgery, etc.)

Operating while weight is still changing may compromise long-term results as they may require more excisional surgeries in the future as they continue losing weight post primary procedure.

Stable weight generally allows:

  • more accurate skin removal 
  • better scar placement 
  • improved symmetry 
  • more predictable outcomes 

Mistake #10: Believing Body Contouring Is always “Cosmetic”

For many patients, excess skin causes genuine medical and functional problems.

These include:

  • recurrent skin rashes 
  • fungal infections 
  • hygiene difficulties 
  • exercise limitations 
  • discomfort in clothing 
  • emotional distress 

Body contouring is often about restoring comfort, mobility and confidence—not simply appearance.

Mistake #11: Neglecting Skin Health

Healthy skin heals better and often produces better surgical outcomes.

Simple measures can help:

  • avoid smoking and vaping 
  • stay well hydrated 
  • consume adequate protein 
  • protect skin from excessive sun exposure 
  • treat vitamin deficiencies when present 
  • maintain stable weight 

Healthy habits remain the best “skin treatment.”

Mistake #12: Expecting One Surgery to Fix Everything

Major weight loss frequently affects the body from head to toe.

Many patients ultimately choose staged procedures, such as:

  • facelift 
  • breast lift with or without implants
  • tummy tuck 
  • arm lift 
  • thigh lift 
  • buttock lift with or without implants
  • gynecomastia surgery with or without pectoral implants
